Short Stature and Growth Hormone Deficiency in POMC Deficiency: An Unexpected Clinical Association.

- "After euthyroidism had been achieved and estrogen priming had been completed, GH stimulation testing with L-DOPA and clonidine showed peak GH levels of 0.52 and 1.0 µg/L, respectively...This case expands the clinical spectrum of POMC deficiency by demonstrating short stature with biochemically confirmed GHD and a notable early growth response to GH therapy. Further studies are needed to clarify the underlying mechanisms and to guide follow-up and management."

Unilateral Nephrectomy: An Effective Temporizing Measure for Refractory Hypertension in ARPKD

- "During this period, she developed severe hypertension refractory to high doses of anti-hypertensives, including amlodipine, diuretics, labetalol, minoxidil, clonidine, and parenteral drips. She continued to require mechanical ventilation but was successfully supported on a conventional ventilator. Palliative unilateral nephrectomy is an effective tool for blood pressure control and temporizing of thoracoabdominal competition, allowing time for growth until renal transplant is feasible. Delaying eventual bilateral nephrectomy is beneficial given concerns surrounding effective dialysis in an anuric neonate."

Management of Status Dystonicus in the PICU: A Scoping Review of Recent Literature

- "There is broad agreement, with some regional variation, on the medical management, with antipyretics, analgesia, sedation (lorazepam), anticholinergics (trihexyphenidyl), muscle relaxants (primarily baclofen and benzodiazepines), alpha-2-adrenergic agonists (clonidine, tizanidine), and the vesicular monoamine transporter (VMAT2) tetrabenazine. Titratable infusions, including midazolam, dexmedetomidine, opioids, and pentobarbital may be needed... Prevention of SD episodes is key. A multidisciplinary, systematic approach to care is essential, with the input of medical, surgical and other subspecialists. Early consideration of interventional approaches in critically ill children with SD is warranted."

Management of Combined Opioid and Medetomidine Withdrawal at an Academic Medical Center

- "The regimen included treatment with dexmedetomidine infusion (an enantiomer of medetomidine), clonidine (an α2 agonist), long- and short-acting opioids, and multimodal symptom management with agents such as olanzapine and ketamine. We reviewed a sample of 61 patients who had 69 ICU admissions for opioid withdrawal from September 2024 to July 2025. Patients with concomitant opioid and medetomidine withdrawal often required multiple days in the ICU to receive high-dose dexmedetomidine for extended periods, along with multimodal symptomatic treatment. Our experience highlights the challenge of managing withdrawal in the rapidly evolving landscape of opioid adulterants and the importance of timely development and implementation of effective treatment protocols."

Guanfacine for Dexmedetomidine Withdrawal

- "Historically, clonidine has been used for this purpose, however its use is limited by cardiovascular effects, namely hypotension... The results of this evaluation reveal a lack of standardized guanfacine dosing strategies in dexmedetomidine weaning, suggesting an opportunity for protocol development. Additionally, the findings suggest an education opportunity for providers on ensuring the appropriate guanfacine formulation is ordered for the planned route of administration. Immediate future directions for this project include a research project designed to evaluate the comparative efficacy of dexmedetomidine weaning strategies."

- "Case: A 62-year-old woman with drug-induced lupus admitted for clonidine-associated angioedema was receiving IV iron dextran for anemia when she developed acute respiratory distress, chest pain, tachycardia, and hypoxemia, requiring epinephrine, steroids, antihistamines and BiPAP. Type II KS results from allergen-triggered, mast-cell-mediated plaque/thrombus disruption causing ischemia with variable, often underdiagnosed presentations. Temporal association with an allergic trigger is key, and further studies should clarify epidemiology and standardize treatment."

- "Oseltamivir was initiated but subsequently withdrawn as symptom duration exceeded 7 days...The patient transferred to the intensive care unit for management of a hypertensive emergency, requiring labetalol infusion, amlodipine, doxazosin, clonidine and furosemide for blood pressure control... This case illustrates a rare but serious complication of influenza infection and highlights the importance of screening for influenza virus in the setting of non-traumatic rhabdomyolysis, even in those without typical symptoms and outside of the usual flu season."
