Bilateral Thalamic Lesion: An Extremely Rare Presentation of Neuromyelitis Optica Spectrum Disorder

- "Tocilizumab was initiated for relapse prevention; however, after three doses, she developed intractable headache, vomiting, and encephalopathy...Her course was complicated by pneumonia, Clostridioides difficile infection, and paroxysmal sympathetic hyperactivity, managed with clonidine and diazepam. She was subsequently transitioned to eculizumab for long-term relapse prevention... This case describes a rare pediatric presentation of NMOSD with internal capsule and sequential bilateral thalamic involvement, initially presenting with hemiplegia and mimicking both stroke and neoplasm, later complicated by intractable paroxysmal sympathetic hyperactivity. While thalamic volume loss and nuclear atrophy are more characteristic of multiple sclerosis, thalamic involvement in NMOSD is rare and typically affects nuclei associated with inflamed pathways—such as the lateral geniculate and ventral posterior nuclei—secondary to optic neuritis or transverse myelitis. A comparative study of clonidine and dexmedetomidine as an adjuvant to levobupivacaine for caudal analgesia in children undergoing below umbilical surgeries: A randomized double-blind controlled trial.

- "Dexmedetomidine (1 µg/kg) added to 0.2% levobupivacaine (1 ml/kg) for caudal block provides prolonged analgesia with better sedation scores when compared to clonidine (1 µg/kg) with 0.2% levobupivacaine (1 ml/kg) for below umbilical surgeries in pediatric patients, without increasing the incidence of adverse effects. Hence, we would recommend the use of 1 µg/kg dexmedetomi-dine as an adjuvant to 0.2% levobupivacaine."

Diltiazem, esmolol, dexmedetomidine, labetalol, and clonidine were effective; dexmedetomidine was associated with reduced MAP the most, and esmolol favored recovery."

- "ABA therapy and clonidine were initiated for behavioral regulation and sleep stabilization...A safety plan was developed with protective restraints and behavior therapy; naltrexone was considered but not initiated... Across cases, overlapping features included intellectual disability, irritability, impulsivity, anxiety, emotional dysregulation, and low adaptive functioning. Behavioral symptoms were often intrinsic to the neurodevelopmental course rather than secondary to acute metabolic instability. These findings underscore that neurobehavioral issues significantly affect quality of life and caregiver burden, often as much as medical complications."
