The Impact of Diclofenac Suppositories on Post-Cesarean Section Pain: A Systematic Literature Review.
(PubMed, Anesthesiol Res Pract)
- " Combination of diclofenac suppositories with other pain relief medicines reduces the need for rescue pain medicines, which are usually opioids such as morphine, meperidine, or pentazocine. The findings also open pathways for further clinical trials to explore the appropriate combinations, dosages, and administration of NSAIDs for specific surgical populations and settings. Future research should focus on standardizing methodologies and addressing risk of bias to enhance reliability of findings related to diclofenac suppository and multimodal analgesia."

Evaluation of the analgesic effect of a combination of oral acetaminophen and oxycodone after extracorporeal shock wave lithotripsy (SWL).
(PubMed, Urologia)
- "The present study aimed to compare the effects of suppository diclofenac (an example of NSAIDs) and oral administration of the combination of oxycodone (an example of an opioid) and acetaminophen in managing pain in patients with renal colic after SWL treatment. The combination of oxycodone and acetaminophen, including a multimodal analgesic approach, can be considered a preferred option in the clinical setting for pain management of patients undergoing SWL. It provides relief for patients as effectively as NSAIDs, with the same safety profile, leading to high patient satisfaction."

Why is the rectal route for NSAIDS favorable for preventing post-ERCP pancreatitis?
(PubMed, Pancreatology)
- "Our findings suggest that the efficacy of rectal diclofenac in PEP prevention relates more to its higher and consistent systemic exposure than its absolute pancreas levels. The implications are that the rectal route provides both systemic and pancreas exposure for the full duration of PEP vulnerability."

(ACG 2024)
- "353 patients were included, of whom 171 (48.4%) received indomethacin and 182 (51.6%) received diclofenac. PEP rates were not significantly different between patients who received indomethacin {n=17 (9.9%, 95%CI=5.9–15.4)} vs. diclofenac {n=9 (4.9%, 95%CI=2.3–9.2)} (p=0.07)."

Diclofenac as Post-ERCP Pancreatitis Prophylaxis: A Cost-Effective Intervention
(ACG 2024)
- "Rectal diclofenac is not commercially available in the U.S. and rectal indomethacin costs continue to rise substantially... There were 415 ERCPs performed at UTMB from January 2023 through January 2024. Selected patients received rectal indomethacin at a dose of 100 mg, while others with minimum/no risk of PEP did not. Indomethacin suppositories currently cost the institution between $216-293 per 50 mg dose totaling $267,436 in expenditure for fiscal year 2023."

SAFETY OF EARLY DISCHARGE POST ENDOSCOPIC RETROGRADE CHOLANGIOPANCREATOGRAPHY (ERCP): A SINGLE CENTRE EXPERIENCE IN SINGAPORE
(UEGW 2024)
- "All patients in this group received 100 milligrams of rectal diclofenac for prophylactically... From this study, it could be concluded that it was safe to allow early discharge for patients who had undergone ERCP, if there is a clear monitoring protocol and discharge plans. The onset of post ERCP pancreatitis tend to be earlier compared to those who developed post ERCP cholangitis. There is female predominance in the post ERCP pancreatitis group."

Efficacy of Diclofenac and Indomethacin for Prevention of Post-Endoscopic Cholangiopancreatography (ERCP) Pancreatitis: A Systematic Review.
(PubMed, Cureus)
- "This study aims to understand the incidence of post-ERCP pancreatitis after using rectal diclofenac and Indomethacin as prophylactic measures. Studies on rectal Indomethacin also yield contradictory results; while some emphasize the drug's large reduction in PEP incidence, especially in low-risk people, others question its efficacy. We need further studies to clarify the remaining uncertainties."

TAP Block Vs Local Anaesthetic Wound Infiltration for Postoperative Analgesia Post-Hysterectomy
(IASP 2024)
- "Postoperatively, participants received multimodal analgesia including regular paracetamol and diclofenac and as required dosing (prn) of intravenous morphine. As a component of multimodal analgesia, TAP block is superior to LWI for postoperative TAH pain management. Specifically, TAP block compared to LWI was associated with: reduced pain scores, reduced opioid consumption; earlier mobilization and improved patient satisfaction. This study, supported the following recommendations:– Routine uptake of TAP blocks for TAH.– Encourage multimodal analgesia to optimize postoperative outcomes.– Consideration of broader utilization of TAP Blocks for all lower abdominal surgeries.– Development of standardized local postoperative analgesia guidelines.– Procurement of equipment and drugs to ensure accessibility including:– Ultrasound scan machine specifically for theatre use.– Include diclofenac suppositories in essential drug list for postoperative use in theatre and ward.–..."

Efficacy and Safety of Rectal Diclofenac and Paracetamol for Pain Control after Caesarean Section
(IASP 2024)
- "Both rectal diclofenac and paracetamol are effective and safe options for use as analgesia after caesarean delivery with adjuvant intramuscular pentazocine to help boost their efficacy.Patient satisfaction following their use is high and results obtained from this study confirms that either of the two protocols is safe for use in setting. As cost is a major consideration in access to quality care in developing countries, these approaches especially the combination of rectal acetaminophen(paracetamol) and pentazocine will be attractive to our clients looking for affordable but effective options for pain relief. These patients often pay for medical services using very limited resources which are required for other competing demands such as feeding, accommodation and education for their children."

Efficacy and safety of diclofenac suppository for postoperative pain relief after diagnostic hystero-laparoscopy and dye test: A double-blind, placebo-controlled, randomized trial.
(PubMed, Eur J Obstet Gynecol Reprod Biol X)
- "Postoperative use of rectal diclofenac and pentazocine is safe, but did not significantly improve pain scores, patient satisfaction and need for rescue analgesia following dHLD, compared with patients who received pentazocine and placebo. While a multi-modal approach to pain relief following dHLD does not appear to be significantly beneficial, a multi-centre study is needed to confirm or refute these findings."

CONCLUSION : A combination of rectal diclofenac, paired with aggressive hydration and PD stenting even for a single PD cannulation, may be more effective in the prevention of PEP than any of the modalities in isolation."
