Effectiveness of Rectal Versus Oral Diclofenac for Perineal Pain Relief following Episiotomy Repair at a Tertiary Hospital in Port Harcourt, Nigeria: A randomized controlled study.

- "The mean time interval (hours) between drug administration and the first urine void was similar in both groups (3.19±3.13 vs. 3.29±3.11, t=0.29, p=0.74), and there was no difference in the requirement for additional analgesia (12.1% vs. 9.1%, χ2=0.32, p=0.57). Diclofenac suppository was more effective in management of perineal pain following episiotomy repair and the participants in the rectal route group expressed more satisfaction than their counterparts who received the oral drug."

- "Despite the increased use of NSAIDs suppositories following the 2020 ESGE guidelines, no significant reduction in PEP incidence was observed. These findings call into question the practical effectiveness of NSAIDs in PEP prevention. Most studies demonstrating a beneficial effect of NSAIDs were conducted in younger females and non-European populations, which may limit their applicability to our cohort."

- "From 2015 onward, rectal diclofenac was administered to all patients as a preventive measure for PEP...There were 3 deaths, 1 related to PEP (0.2%). PS placement effectively reduces severe PEP risk following difficult CBD cannulation and supports high success rates in repeat ERCP, while diclofenac showed no significant additional benefit."

- "The inclusion criteria encompassed studies that featured at least two of the following interventions: pre-procedural administration of 100 mg of rectal diclofenac (D) or indomethacin (I), aggressive hydration utilizing normal saline (NS) or lactated Ringer's solution (LR), 5-french pancreatic duct stent (S), and either a placebo or an active control (C). However, D was the most effective method of prevention of moderate-to-severe PEP in this group. The findings of this network meta-analysis indicate that endoscopists should consider all available prophylactic options when performing ERCP, rather than favoring one over the others."

Future research should focus on standardizing methodologies and addressing risk of bias to enhance reliability of findings related to diclofenac suppository and multimodal analgesia."

- "The present study aimed to compare the effects of suppository diclofenac (an example of NSAIDs) and oral administration of the combination of oxycodone (an example of an opioid) and acetaminophen in managing pain in patients with renal colic after SWL treatment. The combination of oxycodone and acetaminophen, including a multimodal analgesic approach, can be considered a preferred option in the clinical setting for pain management of patients undergoing SWL. It provides relief for patients as effectively as NSAIDs, with the same safety profile, leading to high patient satisfaction."
