Delayed Inflammatory Reaction to BDDE-Stabilized Hyaluronic Acid Fillers Triggered by Hordeolum in a Diabetic Patient

- "Materials & A 61-year-old female patient with type 2 diabetes mellitus (treated with metformin 1000 mg/day) received HA filler injections (Croma®) on October 2, 2024, in the zygomatic area, tear troughs, nasolabial folds, chin, and lips...She was treated with oral amoxicillin-clavulanate (1000 mg BID), ofloxacin eye drops, and topical oxytetracycline... The patient was started on systemic clindamycin (300 mg TID) and ciprofloxacin (500 mg BID). Intramuscular dexamethasone 8 mg was administered once daily for three consecutive days, followed by a tapering course of oral prednisolone: 48 mg/day (5 days), 32 mg/day (5 days), 16 mg/day (5 days), and 8 mg/day (5 days)... Delayed inflammatory reactions to HA fillers are likely immune-mediated and may be triggered by systemic events, even months after injection. Although a hordeolum is typically a localized eyelid infection, it may serve as an immune trigger in susceptible individuals. Diabetic status may predispose..."

Systemic Envenomation via Ocular Route from a Neurotoxic Snake Venom Ophthalmia--A Case Report.

- "Ofloxacin ophthalmic drops and eye lubrication were prescribed for seven days to minimize complications, and the patient was discharged well after three days of hospitalization. Although difficult to ascertain, signs and symptoms pointed to systemic envenomation, and this could be the first case of an unconventional venom route to date. This paper opens possibilities and serves as precedent for future research - ocular envenomation, snake venom, and blood components, and symptomatic management."

- "The current results showed that the typical conjunctival bacteria had higher resistance to fluoroquinolones (although they were not tested specifically to ofloxacin), suggesting a possible selection of resistant bacteria that should not be taken for granted in clinic. However, the same bacteria did not exhibit cross-resistance in the analysis of vancomycin and third-generation cephalosporins. This real-world, longitudinal, prospective study on conjunctival flora analyzed bacterial resistance profiles and contemporary antibiotic use, offering deeper insights into this subject."

To date, this is the first description of a rotation of an artificial iris located in the ciliary sulcus by irrigation of the anterior chamber during GDD implantation."

- "The patient was diagnosed with herpes zoster ophthalmicus (HZO) with concurrent ipsilateral parotitis and subsequently treated with valacyclovir, ofloxacin eye drops, topical erythromycin ointment and amoxicillin/clavulanic acid. Acute Iridocyclitis Associated with Intravenous Zoledronic Acid

- "She'd had 2 prior outside ED visits for these symptoms and received polymyxin B and later ofloxacin eye drops which did not relieve them...She was discharged with olopatadine eye drops and cetirizine, diagnosed with allergic conjunctivitis, and referred to ophthalmology...Right acute iridocyclitis was diagnosed, she received prednisolone eye drops and cyclopentolate eye drops... ZA-associated ocular inflammation is a rare but serious side effect. This adverse effect has been hypothesized to be associated with increased levels of pro-inflammatory cytokines including Interleukin (IL)-1, IL-6, and tumor necrosis factor (TNF)-alpha. As with other acute phase reactions to ZA, it does not appear that ocular inflammation tends to recur with subsequent ZA."
