Management of Postdural Puncture Headache in a Patient on Anticoagulation

- "...Two long cotton-tipped applicators with 5% lidocaine jelly were applied to each nostril and advanced posteriorly until resistance was met...The last Lovenox dose had been given 24 hours prior so an EBP was performed at L4-L5 with 20mL of autologous blood...While this patient reported only mild and temporary relief from the block, it was an appropriate and temporizing option for her at the time due to recent anticoagulation. Hence, topical sphenopalatine ganglion block may a less invasive option for cases in which EBP is contraindicated to relieve PDPH."

- "...Skin prick and intradermal testing were negative to aflibercept, ranibizumab, bevacizumab, and povidine...Due to her positive hypersensitivity testing to 2 ester anesthetics, the patient underwent skin prick and intradermal testing to the amide anesthetic, lidocaine...The culprit allergen, the local anesthetic, could have been overlooked without an assessment of all medications used during the procedure. This case highlights the importance of a thorough allergy evaluation of all medications used during procedures to determine the causative agent.Chief Complaint: Eyelid swelling and rash after ophthalmic procedures for macular degeneration."

- "The technique of eyelid retraction for intravitreal injection has a low rate of endophthalmitis, similar to the reported rates using a metal lid speculum. This is beneficial for both the physician and the patient as it minimises patient discomfort as well as the duration of the procedure. To our knowledge, this is one of the largest studies performed to date evaluating intravitreal injection-related endophthalmitis."

- "...All subjects had used either 6.5% lidocaine spray or 2% topical formulations of lidocaine...Three patients experienced status epilepticus that led to cardiac arrest, and death (all cases with suicidal intention). Based on the results of this study, most cases of topical lidocaine toxicity were among < 40-year-old patients with a male to female ratio of 1.2, with suicidal attempt in 90%, need for intensive care in 36.6%, and mortality rate of 10%."
