Pericapsular nerve block and ablation as a novel pain modifying strategy in sickle cell patients wiht refractory hip pain due to avascular necrosis
(ASH 2025)
- "Beyond duloxetine, aserotonin and norepinephrine reuptake inhibitor (Brandow et al, Blood Adv, 2020), there is little progresson pain management for medication-resistant pain (Okpala et al, J R Soc Med, 2002)...Under ultrasound guidance, the iliopubiceminence was identified and a solution of 0.25% ropivacaine and 40mg of Kenalog was injected at thetarget area using a 25-gauge needle... A two-step approach to mitigating AVN-associated pain can provide sustained pain relief inpatients with sickle cell disease. Specifically, cryoablation offers nerve-sparing sustained pain relief forpatients who are not surgical candidates. Further work is needed to establish AVN-grade specifictreatment approaches for this patient population."

Anterior Quadratus Lumborum Block: A Regional Anaesthetic Technique for the Management of Acute Back Pain in the Emergency Department.
(PubMed, Cureus)
- "Given ongoing severe pain (NRS = 8/10), ultrasound-guided bilateral AQLB was performed using 30 mL of 0.18% ropivacaine on each side with a 21G SonoPlex®II needle (Pajunk®, Geisingen, Germany), accompanied by intravenous dexamethasone (6 mg). This case suggests that AQLB is a feasible and effective opioid-sparing rescue technique for the management of acute back pain in the ED secondary to QL syndrome. It adds to emerging evidence supporting its role as a safe, effective, and resource-efficient intervention, meriting further prospective evaluation."

Impact of transversus abdominis plane block plus intravenous lidocaine on rapid recovery after bariatric surgery.
(PubMed, Front Med (Lausanne))
- "The CG received ultrasound-guided TAPB with ropivacaine, while the SG received ultrasound-guided TAPB with ropivacaine plus intravenous lidocaine. The two groups were compared in terms of total doses of propofol, remifentanil, and sufentanil used; pain intensity; intraoperative conditions; levels of inflammatory factors; additional use of postoperative analgesics; incidence of adverse reactions; and postoperative recovery time...Furthermore, the SG exhibited a shorter time to first flatus, first defecation, and first ambulation, as well as a shorter length of hospital stay compared to the CG (all p < 0.05). TAPB combined with intravenous lidocaine may provide effective postoperative analgesia for patients after bariatric surgery and may accelerate their rapid recovery."

The Effect of Pericapsular Nerve Group Block with Liposomal Bupivacaine on Postoperative Rebound Pain in Older Hip Fracture Patients: A Randomized Controlled Trial.
(PubMed, Drug Des Devel Ther)
- "Ninety patients scheduled for hip fracture surgery were randomized into three groups: LB (liposomal bupivacaine, 30 mL), R (0.375% ropivacaine, 30 mL), and C (saline, 30 mL)...There were no significant differences between the groups in adverse events. LB PENG blockade reduces rebound pain incidence post-nerve block in elderly hip fracture patients, decreases PCA demand, preserves quadriceps function, and enhances satisfaction."

(PubMed, Local Reg Anesth)
- "EOI group received 30 mL of 0.25% ropivacaine bilaterally 30 min before induction of general anesthesia, whereas control group did not receive any block...The plasma levels of NE, Cor, HMGB1, TNF-α and IL-6 at 6 h postoperatively in EOI group were significantly lower than in control group. EOI block given to patients undergoing OLR could provide effective postoperative analgesia, enhance the quality of postoperative recovery, and attenuate postoperative stress and inflammation."

Evaluation of drug distribution, sensory and motor blockade regions in canine coccygeal epidural anesthesia.
(PubMed, J Vet Med Sci)
- "Six dogs received three different doses (0.1, 0.2, and 0.3 mL/kg) of a mixture containing equal volumes of ropivacaine and iohexol, administered via the first and second coccygeal intervertebral spaces...Despite the increased volume at 0.3 mL/kg, greater leakage through the neural foramina resulted in a net epidural dosage comparable to 0.2 mL/kg. These findings indicate that coccygeal anesthesia at 0.1 mL/kg provides SB from the tail to the perineal region while maintaining hindlimb motor function in dogs."
