- "In both patients, morning serum cortisol levels remained elevated following 1 mg dexamethasone suppression test (DST) and 4 mg intravenous DST, while basal adrenocorticotropic hormone (ACTH) levels were suppressed...The first case involved a 69-year-old woman who exhibited significant increases in cortisol in response to luteinizing hormone-releasing hormone (LHRH; +142%), vasopressin (+66%) and metoclopramide (+98%)...In patients with MACS who have an aberrant cortisol response following LHRH stimulation, the long-acting GnRH agonist leuprolide acetate can be used to suppress endogenous LH production and improve hypercortisolism. A greater increase in cortisol secretion following LHRH stimulation and small response to the other stimuli could potentially be predictive of a better response to treatment with leuprolide acetate."

Which Analgesic Should We Use to Relieve Pain After Knee or Hip Arthroplasty? A Systematic Review and Network Meta-analysis of RCTs.

- "For knee arthroplasty, local levobupivacaine appeared to be the most effective option. For hip arthroplasty, local ropivacaine combined with ketorolac and adrenaline appeared the best. In terms of functional improvement, nerve-block levobupivacaine combined with dexmedetomidine (used under monitored conditions) appeared the most effective after knee arthroplasty. Although most interventions had similar safety profiles, intravenous tramadol combined with metoclopramide decreased the risk of nausea. Clinicians should integrate these results with known safety profiles and patient-specific factors to guide individualized postoperative pain management after knee and hip arthroplasties."

Patients with SV CHD, those on elemental formula, tube assisted feeding and those with longer length of stays are more likely to be discharged on anti-reflux or pro-motility medications. An interesting finding is the positive association of the use of these medications with growth velocity prior to discharge specifically among SV patients. This suggests that these infants may have additional benefit from utilization of these medications at discharge."

- "We report a case of gastric POEM (GPOEM) with a mucosal defect closure from inside the tunnel due to difficult luminal side closure.Case Description/ A 48-year-old female with gastroparesis (gastric emptying study with 68% retention at 4 hours) with abdominal pain after eating, epigastric fullness, early satiety, bloating, nausea with rare vomiting, not feeling hungry, and 60 lbs weight loss over two years underwent GPOEM due to concern about long term neurological side effects with metoclopramide in the setting of lamotrigine use for bipolar disorder. Administration of Metoclopramide Prior to Endoscopy for Acute Upper Gastrointestinal Bleeding: A Systematic Review and Meta-Analysis

- "A total of 7 studies were included, resulting in a pooled analysis of 665 patients. Metoclopramide significantly increases the Endoscopic Visualization Score of the Fundus (SMD=0.30, 95% CI: [0.12,0.47 ]; p=0.0008; I²=0%), Endoscopic Visualization Score Body (SMD=0.25, 95% CI: [0.07,0.42 ]; p=0.006; I²=0%), Endoscopic Visualization Score Antrum (SMD=0.23, 95% CI: [0.05,0.40 ]; p=0.01; I²=0%), and Endoscopic Visualization Score Total (SMD=0.32, 95% CI: [0.15,0.50 ]; p=0.0003; I²=0%). Outcomes such as length of hospital stay (p=0.73), re-endoscopy (p=0.53), RBC transfusion units (p=0.42), second look endoscopy(p=0.44), duration of endoscopy (p=0.82), and mortality (p=0.86) showed insignificant results. In conclusion, metoclopramide usage improved the endoscopic visualization, but it showed no significant results when evaluated for the secondary outcomes."

Comparative Analysis of Medication Use and Healthcare Utilization in Gastroparesis and Functional Dyspepsia: A Propensity-Matched Cohort Study
