Analgesic Utility of Bilateral Serratus Anterior Nerve Block Catheters after Bilateral Orthotopic Lung Transplant in the setting of VV-ECMO cannulation and Epidural Contraindication (ASA 2026) - "External oblique and rectus sheath plane blocks with Exparel provided only transient benefit. Subsequent placement of bilateral serratus anterior catheters with ropivacaine infusions for seven days was associated with improved analgesia, reduced ventilator asynchrony, improved respiratory status, successful extubation and ECMO decannulation with chest tube removal."

Recurrent Postoperative Cognitive Dysfunction in a Frail Elderly Patient Undergoing Revision Spinal Fusion Despite Volatile-Free Anesthesia (ASA 2026) - "Previous surgery was complicated by severe year-long postoperative cognitive decline (POCD) so anesthesia was maintained with total intravenous anesthesia (TIVA) using propofol and remifentanil, avoiding volatile agents, and minimizing opioids. Prior low dose ketamine caused "severe agitation, hallucinations and thoughts". Intraoperatively pain was managed with IV Tylenol and surgically placed paraspinal plane, subcutaneous and intramuscular infiltration with a mixture of Bupivacaine/Exparel. Despite these precautions, she experienced recurrent POCD. TIVA is a common strategy to mitigate POCD however it does not eliminate the risk."

Exparel vs. Marcaine for donor site analgesia in atraumatic lower extremity wound STSG: Does neuropathy alter the pain equation? (PubMed, J Plast Reconstr Aesthet Surg) - "Exparel and Marcaine provided superior donor site analgesia compared to lidocaine in patients undergoing STSG for chronic LE wounds. Pre-existing peripheral neuropathy was an independent predictor of higher pain, indicating that patient-specific factors should inform analgesic strategy. These findings support a personalized approach to donor site pain management, particularly in high-risk subgroups."

Understanding the molecular insights of marketed liposomal drugs using molecular dynamics simulations of reduced scale coarse-grained models. (PubMed, NPJ Drug Discov) - "In this study, we employed coarse-grained molecular dynamics simulation to investigate the structural details of the commercially available nine liposomal formulations, with five passive loading systems (Arikayce Kit®, Visudyne®, Ambisome®, Mepact®, and Exparel®) and four active loading systems (Doxil®, Myocet®, Onivyde®, and Vyxeos®)...In four active loading systems, the presence of ions within the liposome cavity promotes the self-aggregation of drugs, thereby enhancing drug retention and release profiles. Dual-tunable liquid crystal-forming system enables sustained ropivacaine delivery for prolonged local analgesia. (PubMed, Mater Today Bio) - "Compared with commercial bupivacaine liposome injection, Rop@LCFS exhibited 8-fold prolongation of effective sensory blockade duration in the rat sciatic nerve block model, while maintaining over 12 h of sustained analgesia in the subcutaneous infiltration anesthesia model. The formulation also displayed favorable biodegradability and biocompatibility both in vitro and in vivo. The Rop@LCFS features easy preparation, sustained release, and biocompatibility, offering potential for clinical translation and long-acting local anesthetics development."
