A solvent-free ropivacaine-loaded composite hydrogel assembled from pH-sensitive micelles and a thermosensitive injectable hydrogel for prolonged local anesthesia.

- "The effective analgesic time of Rop-micelles@HPCH (20.3 h) or Rop-micelles@HPCH-Dex with only a small fraction (<one thousandth) of adjuvant dexmedetomidine (29.1 h) was evidently longer than those of the free Rop HCl (4.2 h) and the commercial EXPAREL® injection (4-6 h) groups in the rat sciatic nerve model. In this study, we develop the ropivacaine-loaded pH-sensitive micelles/hydrogel composite via a solvent-free process based on pH-sensitive and biodegradable polymeric micelles and thermosensitive and injectable hydroxypropyl chitin for postoperative pain management. The drug-loaded micelles/hydrogel composite exhibited sustained drug release in vitro and in vivo, displaying a much longer effective analgesic time than that of the free ropivacaine injection and the commercial EXPAREL® injection in the rat sciatic nerve blockade model."

Nanotechnology for pain management in orthopedic surgery.

- "This narrative review evaluated the potential of nanotechnology platforms (DepoFoam bupivacaine®, DepoDur®, Exparel®, Zynrelef®, NeuroCuple™) in the control of postoperative pain following orthopedic procedures...All EREM dosages diminished the mean fentanyl usage vs placebo and delayed the median time to first dose of fentanyl...Opioid consumption was reduced by 37% in the Zynrelef® PR cohort vs 25% in the bupivacaine HCl cohort. In individuals experiencing total knee arthroplasty and total hip arthroplasty, the use of a nanotechnology-based NeuroCupleTM device diminished postoperative pain at rest by 34% and reduced pain with movement by 18%."
