- "However, the prognostic value of BP-TTR among patients with hypertension (HTN) and coronary artery disease (CAD) remains unclear.Research Question: Is higher BP-TTR over a 6-month period associated with a reduced risk of primary cardiovascular outcome (composite all-cause mortality, nonfatal myocardial infarction, nonfatal stroke) among patients with HTN+CAD? In this post-hoc analysis of the randomized INternational VErapamil-trandolapril STudy (INVEST), we pooled patients (aged ≥50 years with HTN+CAD) randomized to a calcium antagonist or non-calcium antagonist strategy, which had equivalent outcomes in INVEST... In patients with HTN+CAD, higher BP-TTR at the target of <140/90 mmHg may be associated with a reduced risk of the composite outcome of all-cause mortality, nonfatal myocardial infarction, and nonfatal stroke, though statistical significance was not reached after adjustment. We used data from the US cohort of the INternational VErapamil SR-Trandolapril Study (INVEST), a randomized controlled trial of hypertensive patients ≥50 years old with CAD evaluating a calcium antagonist vs β blocker strategy. In a machine learning model applied to a large population of older hypertensive patients with CAD, the top contributors to long-term outcomes were age, race, diabetes, and BP variability."

MACHINE LEARNING PREDICTION MODEL OF BLOOD PRESSURE VARIABILITY

- "We investigated predictors of higher BP variability in International Verapamil SR-Trandolapril Study (INVEST) patients aged ≥50 years with hypertension and CAD and treated with a calcium antagonist vs β-blocker strategy. We used XGBoost machine learning algorithm based on the gradient boosting framework to identify predictors of higher BP variability. In a large population of older patients with CAD, age, baseline & mean BP and sex were the strongest predictors of BP variability. Efforts to identify and minimize visit-to-visit systolic BP variability and its etiology may improve outcomes in this population."

Optimal systolic blood pressure and reduced long-term mortality in older hypertensive women with prior coronary events - An analysis from INVEST☆.

- "We analyzed long-term all-cause mortality data from the International Verapamil SR/Trandolapril Study (INVEST), stratified by risk attributable to clinical severity of CAD (women with prior coronary events of myocardial infarction or revascularization considered high risk, all others at low risk) and by age group (50-64 or ≥65 years)...A similar pattern was observed among low-risk women ≥65 and <65 years old. Among women ≥65 years old with hypertension and prior coronary events, in-trial SBP between 130 and 139 mmHg was associated with lower mortality over the long term versus SBP <130 mmHg."
