OB Hypotension: Spinal Dose after Epidural
(ASA 2025)
- "Forty-five minutes after the last dose of the epidural medication with 9 mL of 0.0625% bupivacaine plus 2 mcg/mL fentanyl, an intrathecal injection was performed with 12 mg of 0.75% bupivacaine with dextrose. Despite immediate intravenous phenylephrine infusion after spinal, the patient experienced severe hypotension (BP 60s/40s), leading to prolonged fetal decelerations and emergent delivery. Patient and newborn recovered without morbidity."

Successful ultrasound-guided epidural anaesthesia following failed spinal anaesthesia in a patient with neurofibromatosis 1.

- "Spinal anaesthesia was attempted using 3 mL of 0.5% hyperbaric bupivacaine (dextrose 8%), resulting in a unilateral block...The next day, ultrasound-guided epidural anaesthesia at T10-T11 was performed successfully with placement of an epidural catheter in situ for postoperative pain control. This is a rare phenomenon underlining the complexity of anaesthetic management in patients with NF1 and significant spinal abnormalities."

Cold exposure did not degrade bupivacaine. A complex and possibly inconsistent reaction involving dextrose was identified that requires further analysis of a larger sample size to elucidate the mechanisms."
