Misalignment in circadian-regulated glymphatic clearance as a driver of neurodegeneration
(AAIC 2023)
- "Aquaporin-4, α-syntropin, Connexin-43 as well as clock genes expression have been assessed in colon, and in different brain areas, by real-time PCR... The characterization of the cascade of events and the relationship among gut inflammation, glymphatic influx alteration and brain waste product deposition has a novel significance since it will allow understanding how a persistent peripheral enteric hit may disorganize a central hierarchy thus triggering a self-sustaining process prodromal of neurodegeneration."

[VIRTUAL] An Adult With Hair Loss! a Rare Case of Non-Classical 3β Hydroxysteroid Dehydrogenase (3β HSD) Deficiency.
(ENDO 2021)
- "Co-syntropin stimulation test has been delayed due to the pandemic...The normal levels of estradiol and estrone are likely due to peripheral conversion of androstenedione by aromatase. 3β HSD deficiency is usually treated with steroids which helps resolve most symptoms."

Evaluation of the glucocorticoid, mineralocorticoid, and adrenal androgen secretion dynamics in a large cohort of patients aged 6-18 years with transfusion-dependent β-thalassemia major, with an emphasis on the impact of cardiac iron load.

- "Patients with thalassemia have a high prevalence of AI due to HC and zona glomerulosa and zona reticularis hypofunction. TBIL surrogates can predict ACH, but cardiac iron was the only surrogate that was adequately sensitive to predict the severity of ACH."

Pain Med
- "It is reasonable to consider IV cosyntropin as the treatment of choice for patients in whom EBP is contraindicated or in austere environments where there is limited or no access to anesthesia trained providers. Future research should compare efficacy and cost of prophylaxis to treatment of PDPH with intravenous cosyntropin and evaluate the most effective dosing regimen, including duration, number, and strength of doses."

A NOVEL TMEM127 GENE MUTATION ASSOCIATED WITH BILATERAL PHEOCHROMOCYTOMAS AND ATYPICAL SYMPTOMS
(AACE 2018)
- "...Plasma blood work revealed elevated metanephrines 1390 pg/ml (12-67) and normetanephrines 601 pg/ml (18-101); ACTH 7 pg/nl (8-42) with baseline cortisol of 6.6 ug/dl that peaked to 14.2 ug/dl at 60 minutes after 250 mcg injection of co-syntropin...She was started on phenoxybenzamine and hydrocortisone with subsequent bilateral partial adrenalectomy that was uneventful and resolved her symptoms... We describe a novel TMEM127 mutation associated with bilateral PCCs. Mutations involving the initiator codon of Exon 2 in the TMEM127 gene are known to be pathogenic. Individuals with these mutations are usually asymptomatic or present with typical adrenergic symptoms which was not the case with our patient."

Undiagnosed Addison's Disease as a Cause of Infertility
(ENDO 2019)
- "...Elevated 17-OHP was thought to represent luteal phase, but repeat in early follicular phase remained elevated, prompting referral to Endocrinology.Co-syntropin stimulation test was performed with pre-ACTH 17-OHP persistently elevated (546 ng/dl), with minimal increase 1 hour post-ACTH (598 ng/dl)...Following diagnosis of AD, HC (hydrocortisone) 10 mg in the morning and 2.5 mg in the afternoon was initiated with improvement in menstrual regularity to 34 day cycles. RE recommended initiation of letrozole for ovulation induction, and intrauterine insemination...This case emphasizes the importance of a thorough endocrinologic evaluation in patients with infertility, particularly those with other autoimmune endocrinopathies. The etiology of infertility in AD is likely multifactorial, and there is minimal supportive literature to date. While some authors have proposed LFOR secondary to adrenal hypoandrogenism or POF as an underlying etiology, this unique case describes a..."
