Determining Reintubation Risk Factors and Effects on Outcomes in the Surgical ICU.
(PubMed, J Intensive Care Med)
- "In the full-cohort model, atrial fibrillation, spinal cord injury, alcohol withdrawal, ventilator-associated pneumonia (VAP), combined propofol-dexmedetomidine sedation, and admitting service were independently associated with RI; VAP timing relative to RI was uncertain...Patient-specific risk assessment may help guide risk-stratified post-extubation monitoring and respiratory support. The exploratory subgroup findings require prospective validation."

Effect of Early Dexmedetomidine Addition on Prognosis in Mechanically Ventilated Patients Under Propofol Sedation: A Target Trial Emulation Study.
(PubMed, Ann Pharmacother)
- "In mechanically ventilated patients receiving propofol sedation, early initiation of DEX was associated with a reduced risk of 28-day mortality but increased odds of bradycardia and severe hypotension. These findings indicate that the potential benefits of early DEX use should be carefully weighed against its cardiovascular safety risks in clinical practice. Further prospective studies are warranted to validate the present results."

Comparison of analgesic efficacy of dexamethasone and dexmedetomidine as an adjuvant to local anaesthetic in transversus thoracic muscle plane block in adult patients undergoing median sternotomy: A randomised controlled trial.

Comparison of three sedation regimens for complex bronchoscopic procedures: an observational study on efficacy, safety, and satisfaction.
(ERS 2026)
- "Aims: To compare safety, efficacy, and satisfaction of: 1) Midazolam+Fentanyl (M+F), 2) Dexmedetomidine+Fentanyl (D+F), and 3) NORA. D+F is as safe and effective as M+F, significantly reducing rescue benzodiazepine needs. Importantly, D+F empowers pulmonologists to autonomously manage complex cases with high stability, providing a key alternative to NORA. While NORA remains the gold standard for patient comfort, it involves longer procedural and recovery times."

Sedation strategy and ventilator liberation in acute respiratory failure: A multicenter analysis
(ERS 2026)
- "Early benzodiazepine exposure is associated with increased failure to liberate from mechanical ventilation, whereas benzodiazepine sparing strategies particularly dexmedetomidine are associated with more favorable ventilator liberation trajectories after accounting for competing mortality risk."

Efficacy and safety of sedation induced with cipepofol alone versus a dexmedetomidine plus propofol regimen for patients requiring mechanical ventilation: a randomized, open-label, non-inferiority trial.
(PubMed, Ann Intensive Care)
- "Furthermore, cipepofol elicited fewer hypotensive events than dexmedetomidine/propofol. Large-scale, multicenter studies are warranted to confirm our findings."

Short-term outcomes across sedation practice patterns in mechanically ventilated patients with bacterial pneumonia: a nationwide cohort study.
(PubMed, Ann Intensive Care)
- "Tracheostomy was more frequent with combined sedation than with both comparators, whereas 28-day ventilator-free days did not differ. Combined sedation was not associated with a lower risk of the composite outcome of in-hospital death and functional decline at discharge, compared with propofol-based sedation; however, a lower risk was observed compared with dexmedetomidine-based sedation."

Renoprotective Effects of Dexmedetomidine Versus Dopamine in High-Risk Renal Patients Undergoing Off-Pump Coronary Artery Bypass Grafting:A Prospective, Randomized, Comparative Study.
(PubMed, J Cardiothorac Vasc Anesth)
- "Dexmedetomidine infusion was superior to dopamine in improving urine output and eGFR in high-risk renal patients undergoing OPCAB, although differences in serum creatinine and BUN were not clinically significant. Neither drug prevented AKI beyond the control group. Dexmedetomidine may offer a favorable renal physiological profile in this patient population, warranting further investigation with more sensitive biomarkers."

Identification of immune key genes in sepsis and the regulatory effects of dexmedetomidine based on transcriptome analysis and machine learning
(PubMed, Xi Bao Yu Fen Zi Mian Yi Xue Za Zhi)
- "The results demonstrated that DEX treatment reduced the expression of sepsis-related proteins, including KCNMA1, cyclin B2 (CCNB2), and NEDD4 E3 ubiquitin protein ligase (NEDD4), and suppressed inflammatory cytokine levels by downregulating KCNMA1. Conclusion DEX suppresses the development of sepsis by downregulating KCNMA1 expression."

Awake Fiberoptic Intubation for Segmental Mandibulectomy in a Patient with Follicular Ameloblastoma of the Anterior Mandible: A Case Report.
(PubMed, Int Med Case Rep J)
- "Topical anesthesia was achieved by combining nebulized 2% lidocaine (4 mL, 80 mg) with the spray-as-you-go technique (2-4 mL of 2% lidocaine)...Sedation was achieved with dexmedetomidine (loading dose 1 µg/kg over 10 minutes, then maintenance at 0.4 µg/kg/hour)...No episodes of desaturation occurred. This report highlights the practical adaptability of the DAS sTOP framework for AFOI when HFNO availability is limited, demonstrating that low-flow nasal oxygen combined with dual-method topicalization and a single sedative agent may serve as a feasible alternative."

Comparison of the efficacy of different interventions for rebound pain after brachial plexus block in upper extremity surgery: A systematic review and network meta-analysis.
(PubMed, Eur J Anaesthesiol)
- "In patients undergoing upper-extremity surgery with brachial plexus block, current network meta-analysis findings suggest that continuous nerve block is associated with a lower incidence of rebound pain and reduced pain scores at 12 and 24 h postoperatively, although further validation is still required. We recommend that continuous nerve block may be considered for patients at high risk of rebound pain to balance efficacy and safety. Given the limited certainty of evidence, these findings should be regarded as exploratory and hypothesis-generating rather than definitive conclusions."
