Fetal hemoglobin dysregulation in inherited and acquired bone marrow failure syndromes
(ASH 2025)
- "Introduction: Inherited & acquired bone marrow failure syndromes (BMFS) have "stress erythropoiesis" with macrocytic anemia, elevated fetal hemoglobin (HbF) & erythropoietin (EPO) levels... Fetal hemoglobin is elevated in both patients with inherited & acquired (aplastic anemia) BMFS. When compared to established variables, elevated HbF has neither diagnostic utility nor prognostic relevance in predicting adverse outcomes. On the other hand, onset of cytopenias and presence of a minor PNH clone are both useful clinical variables in differentiating patients with inherited from acquired BMFS."

- "Based on our data, EPO might be used for thrombocytopenia supportive therapy in congenital amegakaryocytopoiesis. Furthermore, since a distribution skewed towards large platelets is an independent risk factor and a poor prognosis indicator in atherothrombosis, the characterization of EPO's role in the production of large-size platelets, if confirmed in human, may open new perspectives in the understanding of the role of EPO-induced platelets in atherothrombosis."
