Double Trouble… or Double Triumph? Dual Biologic Therapy in Refractory Hidradenitis Suppurativa
(AAD 2026)
- "Adalimumab, secukinumab, and bimekizumab are currently FDA-approved for moderate-to-severe HS, with adalimumab demonstrating response rates of 42–59%...He had minimal response to doxycycline, benzoyl peroxide wash, and clindamycin, and subsequently failed adalimumab, secukinumab, acitretin, dapsone, cefadroxil, prednisone, laser hair removal, and topical agents over two years. Given inadequate disease control, infliximab was added to ongoing secukinumab... Dual biologic therapy may represent a promising option for patients with severe, refractory HS unresponsive to monotherapy with currently available systemic treatments, including FDA-approved biologics used individually. Larger studies are needed to clarify long-term efficacy and safety."

Postoperative Extended Oral Antibiotic Prophylaxis Continues to Increase in Prevalence with Questionable Improvements in One-Year Periprosthetic Joint Infection Rates
(AAOS 2026)
- "All patients who underwent primary THA or TKA from January 1 st, 2016 to April 1st, 2024, had osteoarthritis as their primary diagnosis, and were given IV Ancef pre-operatively were included. We evaluated all patients who received at least 48-hours of oral cefadroxil or cephalexin and compared them to all patients who did not receive extended oral antibiotics postoperatively... The use of extended oral antibiotic prophylaxis following primary THA and TKA continues to increase in popularity, especially in high-risk patients. However, this large database study fails to demonstrate improvements in one-year PJI rates with EOAP in high-risk patients with obesity, diabetes, and CKD."

Structure-dependent transformation of cephalosporin antibiotics at MnO2 surface mediated by iodide and formation of highly toxic iodinated products.
(PubMed, Water Res)
- "These RIS facilitate the degradation of cefoperazone (CFP) and cefadroxil (CFX) through electrophilic substitution at phenolic hydroxyl groups, while their formation competes for the active sites at MnO2 surface, thereby hindering the direct oxidation of cefalexin (CEX) and cefazolin (CFZ) that lack such reactive moieties. Toxicity assessments using luminescent bacteria test and ECOSAR predictions indicate that the I-TPs exhibit higher toxicity than the parent compound and its non-iodinated products. These findings underscore that reactions between MnO2 and I- can profoundly alter the environmental fate and potential toxicity of cephalosporins in iodine-rich water environments."

Amoxicillin-induced Erythema Scarlatiniforme Desquamativum Recidivans with cross-reactivity to cefadroxil in a child: A case report and review of literature.
(PubMed, Br J Clin Pharmacol)
- "This case establishes the first report of generalized ESDR triggered by amoxicillin in a paediatric patient. It emphasizes a unique pattern of drug hypersensitivity, characterized by cross-reactivity to cefadroxil and tolerance to other tested penicillins (phenoxymethylpenicillin and oxacillin). It is essential to conduct a complete allergy work-up, which includes OPTs, to conduct a customized cross-reactivity assessment. This assessment is essential for the safe guidance of future therapeutic decisions in patients with drug-induced ESDR."

Primary Genital HSV-2 Infection in a Resource-Limited Setting: Diagnostic Challenges and Clinical Implications.
(PubMed, Cureus)
- "Treatment with oral acyclovir, cefadroxil, and mefenamic acid, supplemented by topical gentamicin and counselling on recurrence risk and safe sexual practices, led to complete lesion resolution within 14 days and no recurrence at six-week follow-up. These limitations hinder accurate case identification and contribute to the underrecognition of HSV-2 prevalence. Early clinical recognition, prompt antiviral initiation, and strengthened patient education remain essential for mitigating morbidity, preventing recurrence, and reducing transmission in low-resource environments."

Bacillus mycoides bacteremia after dog bite trauma in a patient on chronic suppressive antibiotics.
(PubMed, IDCases)
- "The patient, who had a chronic Left Ventricular Assist Device (LVAD) in place and was on a chronic suppressive regimen of doxycycline and cefadroxil for a preexisting driveline infection, developed B. mycoides bacteremia despite ongoing antibiotic therapy. She was successfully treated with intravenous ampicillin-sulbactam plus vancomycin, followed by oral ciprofloxacin upon discharge. The patient's recovery was uncomplicated and the final susceptibility testing confirmed pan-susceptibility to all tested agents. This case highlights the potential pathogenicity of B. mycoides in humans and raises questions on the risk factors for infection and optimal management of B. mycoides infections in the setting of relatively limited clinical data."

Pro-inflammatory secreted virulence factors of Staphylococcus capitis causing a rare occurrence of severe native hip joint infection.
(PubMed, ASM Case Rep)
- "The patient was treated with intravenous cefazolin (6 weeks), after which he was transitioned to oral cefadroxil. The organism secreted a neutrophil pro-inflammatory autolysin protease and an analog of the immunodominant antigen B of S. aureus, previously with no known biological function. Our data suggest that the inflammatory lesion in the patient's right hip was due to S. capitis subspecies capitis, and furthermore, it is likely that two secreted proteins (autolysin and an analog of immunodominant antigen B) contributed to the inflammation."

Medical Management of an Infusion Site Extravasation Injury After Peripheral Intravenous Infusion of Ado-Trastuzumab Emtansine in a 38-Year-Old Woman with Breast Cancer: A Case Report.
(PubMed, Am J Case Rep)
- "She was treated with a topical steroid (clobetasol), topical calcineurin inhibitor (tacrolimus), oral antibiotic (cefadroxil), high-dose vitamin D (ergocalciferol), and pain management (gabapentin) with almost complete resolution in 4 weeks. CONCLUSIONS This case report describes a T-DM1 skin extravasation treated with multimodal medical measures as opposed to a more conservative approach. We reaffirm that the use of port access when administering T-DM1 is critical to prevent extravasation, and we recommend multimodal management, including antibiotics and topical therapies, to prevent infection and expedite wound healing if extravasation occurs."
