Expanding Fentanyl Analog Testing to Include Brain Specimens: Method Validation and Homogenate Stability Assessment.

- "XXXXX recently added brain to its fentanyl analog testing method for 14 analogs (fluoroisobutyryl fentanyl, acetyl fentanyl, acryl fentanyl, alfentanil, butyryl fentanyl, carfentanil, fentanyl, para-fluorofentanyl, furanyl fentanyl, methoxyacetyl fentanyl, norcarfentanil, norfentanyl, sufentanil, and valeryl fentanyl) and 3 U-series drugs (U-47700, U-48800, and U-49900). Brain homogenate stability was improved when frozen and was not impacted by the addition of 1% sodium fluoride. The study herein provides insight into the feasibility of testing brain for fentanyl analogs and their stability under various storage conditions, contributing valuable data to the limited literature on brain toxicology testing."

- "Compared with baseline values at T0, the MAP of pediatric patients in both groups decreased at T2 (P<0.05), but the reduction remained within 20% of the baseline values. Low-dose alfentanil (5 μg kg-1) significantly reduces the ED50 and ED95 of remimazolam for successful loss of consciousness during paediatric general anaesthesia induction and decreases the incidence of adverse reactions during remimazolam induced sedation."

Comparison of Ciprofol Versus Propofol for the General Anaesthesia During Gynecological Day Surgery: A Prospective, Randomized, Double-Blind, Non-Inferiority Trial.

- "Anesthesia induction in the ciprofol group: ciprofol 0.5 mg/kg was given intravenously within 1 minute, alfentanil 20 μg/kg was given intravenously within 30 seconds, mivacurium 0.2 mg/kg was given intravenously within 30 seconds, and anesthesia maintenance: ciprofol 1.25 mg/kg/h combined with alfentanil 40 μg/kg/h was given intravenously. However, the awakening time of the ciprofol group was significantly longer than that of the propofol group (7.57 vs 5.52 min). Ciprofol demonstrated non-inferior efficacy to propofol for gynecological day surgery anesthesia, while offering superior hemodynamic stability and reduced adverse effects."

- "These patients were treated with medically approved novel synthetic opioids (fentanyl, remifentanil, sufentanil, or alfentanil) and traditional opioids (morphine, methadone, and hydromorphone) during their clinical treatment. The metabolite ratio of β-hydroxyfentanyl to fentanyl was particularly well suited for children and neonatal patients. In conclusion, concentration ranges, metabolite ratios, and dose correlations of the studied opioids in pediatric hair were established, providing insights into opioid incorporation pathways."

Targeted use of dexmedetomidine-based combinations was found to be particularly effective across various surgeries, while fentanyl and propofol-midazolam combinations excelled in specific contexts. This study underscores the importance of tailoring anesthetic adjuvant strategies to specific surgical sites to reduce the risk of ED in pediatric patients, and provides a valuable reference for optimizing anesthetic care in this vulnerable population."
