It emphasizes a unique pattern of drug hypersensitivity, characterized by cross-reactivity to cefadroxil and tolerance to other tested penicillins (phenoxymethylpenicillin and oxacillin). It is essential to conduct a complete allergy work-up, which includes OPTs, to conduct a customized cross-reactivity assessment. This assessment is essential for the safe guidance of future therapeutic decisions in patients with drug-induced ESDR."

From surveying ACE unit nursing, physician, and pharmacy staff, frequently reported barriers to allergy assessment and de-labeling included time and staffing constraints, and patient complexity. Our pharmacy-driven interdisciplinary penicillin allergy de-labeling initiative is effective and safe for eligible hospitalized older adults with low PEN-FAST scores (≤1)."

Nocardia Otomastoiditis in a Chemotherapy-Induced Neutropenic Patient With Diffuse Large B-cell Lymphoma: Successful Medical Management.

- "She had a 15-year history of intermittent otorrhea, previously treated with oral amoxicillin-clavulanate with transient improvement. Computed tomography (CT) imaging demonstrated otomastoiditis without bony erosion or intracranial extension. The patient was treated with a brief empiric course of intravenous imipenem, followed by eight weeks of oral TMP-SMX, resulting in the complete resolution of clinical symptoms and significant radiologic improvement. This case highlights the importance of considering Nocardia infection in patients with chronic or refractory otorrhea, particularly in the setting of chemotherapy-induced neutropenia. An early microbiologic diagnosis and focused therapy can lead to complete healing without surgery or long-term use of multiple drugs."

Successful treatment of pulmonary melioidosis in an Australian dog.

- "Antimicrobial therapy included amoxicillin clavulanic-acid (AMC) (day 1-21, then day 28-84), trimethoprim sulfadiazine (day 8-28), enrofloxacin (day 2-16) and doxycycline (day 8-16). There was no evidence of recurrence during the follow-up period, and the dog died of unrelated causes 785 days after the original presentation. This case is the first description of successful treatment of canine pulmonary melioidosis in a region not previously associated with high endemicity."

Although probiotics are generally considered safe, this case demonstrates that L. acidophilus can act as a pathogen in high-risk patients. Necrotizing infection due to L. acidophilus is exceptionally uncommon but should be recognized as possible, particularly in immunocompromised hosts."

- "Individual trials showed marked reductions in post-extraction bacteremia surrogates with chlorhexidine mouthwash prophylaxis and with intravenous amoxicillin-clavulanate in extraction settings, whereas in uncomplicated extractions among well-controlled type 2 diabetes, antibiotics did not reduce postoperative complications and were associated with greater analgesic consumption...In parallel, microbiological evidence indicates that single-dose prophylaxis can promote short-term ecological disturbance and selection of resistant oral flora. Future trials should standardize endpoints (distinguishing clinical infection/implant failure from surrogate bacteremia measures), stratify by baseline risk, and prioritize antimicrobial stewardship to identify the limited patient subsets most likely to benefit."

Infectious mononucleosis with ampicillin rash and a positive rapid streptococcal antigen test: a case report.

This case underscores the diagnostic overlap between IM and streptococcal pharyngitis, the need for caution in interpreting rapid antigen tests, and the importance of avoiding unnecessary antibiotics in suspected viral illness."

A Mysterious Case of Autoimmune Hepatitis Triggered by Herpes Zoster Virus Versus Drug-Induced Liver Injury.

- "While evaluating a patient with suspected autoimmune hepatitis, common causes such as viral hepatitis, Epstein-Barr virus, and medications like amoxicillin-clavulanic acid, statins, nitrofurantoin and methyldopa must be ruled out. She was observed with findings typical of cholestatic liver disease, and an extensive workup, including imaging and liver biopsy, was done to determine the cause of liver injury. Her clinical presentation, laboratory and Imaging findings favored Herpes Zoster virus as the likely culprit for autoimmune hepatitis."

A rare case of Streptomyces fradiae hand actinomycetoma.

- "A skin biopsy of the lesion confirmed the diagnosis of actinomycetoma, and the patient was treated with ceftriaxone IV for 14 days followed by oral amoxicillin clavulanic acid for a total of 3 months. The patient had significant clinical recovery with no recurrence of the infection. Streptomyces fradiae is a rare causative agent of actinomycetoma, and this case highlights the importance of early diagnosis and prolonged antibiotic therapy in the management of this infection."

- "LTCF residence was independently associated with higher AMR in several common urinary pathogens. These findings suggest that empirical UTI regimens commonly used in community-dwelling older adults may be inadequate for LTCF residents. Setting-specific antibiograms, improved diagnostic stewardship, and strengthened AMS interventions are needed to support appropriate prescribing. Further studies incorporating patient-level clinical factors are warranted to refine empirical treatment recommendations for older adults across care settings."
