Massive Hepatic Necrosis in HELLP Syndrome
(ATS 2025)
- "Pertinent labs on presentation included: ALT 2,900 U/L AST 6,820 U/L, total bilirubin 1.9 mg/dL, INR 0.97, and Cr 2.79. Hepatic necrosis is a recognized catastrophic complication of HELLP syndrome, its occurrence postpartum is rare, and management requires a multidisciplinary approach and early referral to a liver transplant center. Few cases in the literature describe conservative management of hepatic necrosis, highlighting the need for further research to guide management without resorting to liver transplantation."

ERBB4 upregulation in a taxol-resistant triple-negative breast cancer cell line

- "Treatment of the cells with ERBB inhibitors, sfatinib, allitinib, or PF299804, in sub-lethal concentration only induced a small additive effect on paclitaxel cytotoxicity in MDA-MB-231 cells, but dramatically enhanced paclitaxel cytotoxicity in T50RN cells. These results suggest that ERBB4 may play a critical role in paclitaxel resistance in T50RN cells. Further elucidation of how ERBB4 is induced in T50RN cells and how ERBB4 regulates cell resistance might provide leads to overcome paclitaxel resistance."

- "OCA treatment led to a statistically significant decrease in aspartate aminotransferase (AST; AST baseline: 38.50 [26.25; 50.00] IU/L vs. AST 6-month follow-up: 23.50 [21.50; 44.25] IU/L, p = 0.0012), alanine aminotransferase (ALT; ALT baseline: 55.50 [28.75; 97.00] IU/L vs. ALT 6-month follow-up: 36.50 [28.00; 57.25] IU/L, p = 0.0035), and gamma-glutamyl transferase (GGT; GGT baseline: 168.00 [100.30; 328.50] IU/L vs. GGT 6-month follow-up: 88.00 [44.50; 259.80] IU/L, p = 0.0063), while the decrease in alkaline phosphatase (AP) was not statistically significant (AP baseline: 197.00 [170.00; 253.30] IU/L vs. AP 6-month follow-up: 196.00 [134.00; 227.00] IU/L, p = 0.0915). In none of our patients did therapy with obeticholic acid have to be stopped due to pruritus or poor tolerability. - "AST 6.6U/L, ALT 8.8 U/L, Urea 29.5 mmol/L, Crea 602 umol/L, LDG 1112, aPTT- 44.6; D-Dimer - 1.53 mg/l, рН- 7.34; рСО2- 44.1; рО2- 61.6; НСО3- 23.4...Two doses of Rituximab (500mg) were administered, and the patient was discharged home...Conclusions In this report, we describe a child with progressive renal failure and pathology exhibiting a membranoproliferative pattern. We thus conclude that this patient exhibited an unusual presentation of chronic thrombotic microangiopathy mimicking MPGN Type II."

CARDIOGENIC SHOCK AFTER VENLAFAXINE OVERDOSE

- "CASE PRESENTATION: A 21-year-old female with history of depression and no known cardiac disease was brought to the ED after ingestion of 14,100 mg of venlafaxine extended-release in a suicide attempt...Lab work showed elevated transaminases (ALT 3,914, AST 6,132), and elevated troponin (72 ng/L)...Three hours later, she had a tonic-clonic seizure, so lorazepam was administered and she was emergently intubated for airway protection...Dobutamine infusion was started and echocardiogram confirmed left ventricular dysfunction with ejection fraction (EF) of 35-40%... As venlafaxine is commonly prescribed for depression, patients and physicians should be aware of potential serious cardiac complications at higher doses."

- "Our results indicate that allitinib was more effective than afatinib in NSCLC cell lines. KRAS mutations increased aggressive behavior through upregulation of the focal adhesion-PI3K-Akt-mTOR-signaling in NSCLC cells. Significantly, everolimus restored sensibility and improved cytotoxicity of EGFR inhibitors in the KRAS mutant NSCLC cell lines."

The implication of RKIP loss in primary resistance to EGFR inhibitors in NSCLC: a novel predictive biomarker?

- "The role of RKIP in the response to EGFR inhibitors was evaluated in vitro through cytotoxicity assays (MTS) and real-time cell analysis (xCELLigence), using Erlotinib, Afatinib, Allitinib and Osimertinib drugs. When assessing the activation levels of EGFR related signaling pathways upon treatment, AKT pathway was overactivated in RKIP KO cells, thus shedding light on the possible mechanism of resistance behind RKIP’s loss. Conclusion To sum up, these results show the potential of RKIP as a negative predictive biomarker to EGFR targeted therapy response in NSCLC."

- "Among them, targeted drugs for HER-2 included trastuzumab, pertuzumab, T-DM1, RC48-ADC, lapatinib, pyrotinib, allitinib, sipatinib, seratinib. The median overall survival time of the two groups was 24.9 months and 40.4 months, respectively, with statistical significance (P<0.001). Primary resistance to HER-2 is one of the factors of poor prognosis in HER-2 positive breast cancer, and its mechanism needs to be further explored."

- "Median peak lab values for APAP and IH cases respectively were similar: APAP: AST 6,491 IU/L, ALT 4,553 IU/L, Bili 4.5 mg/dL; Ischemia: AST 4930 IU/L, ALT 3226 IU/L, Bili 3.15 mg/ dL . The interchangeable clinical scenarios (found unresponsive with high ALT) make it difficult to reliably distinguish APAP from IH . Presence/absence of serum APAP adducts resolves many (xx) of these cases. We identified 6% with combination injuries (APAP or ischemia plus DILI, etc.) and an additional 1% with apparent combination APAP/ Ischemia injury."

- "Soluble phlorotannin levels remained stable during the kinetics and increased significantly only after 12 h in the presence of grazers, compared to the control, before decreasing to the initial steady state for the rest of the kinetics. Under grazing conditions, the expression of vbpo, cyp450 and ast6 genes was upregulated, respectively, at 6 h, 12 h and 24 h, and cyp450 gene was downregulated after 72 h. Interestingly, the pksIII gene involved in the synthesis of phloroglucinol was overexpressed under grazing conditions after 24 h and 72 h. This study supports the hypothesis that phlorotannins are able to provide an inducible chemical defense under grazing activity, which is regulated at different stages of the stress response."

A phase I study of AST1306, a novel irreversible EGFR and HER2 kinase inhibitor, in patients with advanced solid tumors

- P1, N=71; “During dose escalation, the key DLT was grade 3 diarrhea observed in 5 patients at 1000 mg BID (n = 1), 1500 mg BID (n = 1), 800 mg TID (n = 1) and 1200 mg TID (n = 2). AST1306 was rapidly absorbed and had moderate to high clearance….Of the 55 evaluable patients, 7 patients experienced partial responses, including 5 with breast cancer, 1 with lung cancer, and 1 with gastric cancer. The best response with stable disease for >= 6 months was achieved in 7 patients.”

In sum, we experimentally evidenced a hidden human functional proteome encoded by purported lncRNAs, suggesting a resource for annotating new human proteins."
