A review of pediatric chronic hand eczema with a focus on risk factors, allergens, and management
(EADV 2026)
- "Topical calcineurin inhibitors and phototherapy offer steroid-sparring alternatives, while topical JAK inhibitors (e.g., delgocitinib) represent a promising emerging class with active trials in adolescents...Alitretinoin, methotrexate, and cyclosporine are used cautiously off-label. Dupilumab, approved for pediatric atopic dermatitis (AD), has emerged as a favoured first-line systemic therapy for P-CHE demonstrating efficacy and safety...Existing data is primarily derived from limited subgroup analyses within adult predominant trials, or from patients with atopic hand dermatitis enrolled within larger AD studies. Future research must address these gaps to inform guideline development and prioritize international consensus, with the goal of improving patient outcomes."

Rapid Improvement with Topical Delgocitinib in Severe Refractory Atopic Dermatitis: A Case Report
(EADV 2026)
- "The patient was first treated with dupilumab, which resulted in partial improvement but persistent active lesions and pruritus. Therapy was subsequently switched to the oral JAK inhibitor abrocitinib, leading to an initial clinical response followed by secondary loss of efficacy...Conclusion The addition of topical delgocitinib to systemic upadacitinib therapy resulted in rapid and clinically significant improvement in a patient with severe refractory AD. This case supports further investigation of combined topical and systemic JAK inhibition as a potential therapeutic strategy in difficult-to-treat AD."

Practice patterns and educational needs in chronic hand eczema management: a national survey of 137 French dermatologists
(EADV 2026)
- "The instrument explored five domains: demographics and CHE caseload; diagnostic workup (clinical features, patch testing, referral); clinical phenotypes and triggers; severity assessment and prescribing patterns (topical corticosteroids [TCS], alitretinoin, topical delgocitinib, off-label calcineurin inhibitors, phototherapy, ciclosporin, methotrexate, biologics, oral JAK inhibitors); and self-reported educational needs. Conclusions French dermatologists' diagnostic practices are well aligned with European standards, while three priority areas emerge, complementing the previously published CHEMIN survey (Bernier et al., 2025): limited awareness of the ESCD/EuroGuiDerm guidelines, marked under-use of patient-reported outcomes such as the DLQI, and an evolving therapeutic landscape in which the choice between long-term TCS use and earlier introduction of novel topical agents remains driven by individual practitioner familiarity. These findings support the development..."

Pediatric-Origin Disease, Adult-Valued Targets: A Systematic Review of the DELTA TEEN Paradigm in Chronic Hand Eczema
(EADV 2026)
- "Introduction Topical pan-JAK inhibitor that is Delgocitinib cream was found to be effective in treating from moderate to severe chronic hand eczema (CHE) in adolescents in the recently published study of DELTA TEEN...This can lead to more persistent dosing that changes overall long-term safety concerns. Because of this, it's not enough to just use adult dosing and schedules, in the pediatric patients they need their own dosing measures and treatment timelines that take into account with their particular patterns of drug response and usage."

Hand eczema and its subtypes
(EADV 2026)
- "Finally, a stepped-care management approach will be outlined, from prevention and patient education to topical therapy (topical corticosteroids and calcineurin inhibitors), before advancing to therapies for refractory cases (phototherapy, alitretinoin, oral corticosteroids, cyclosporin, azathioprine, methotrexate, acitretin) and the topical pan-JAK inhibitor delgocitinib. Investigative work-up and exposure identification: participants will focus on patient history and diagnostic tools (patch testing, prick testing) to detect relevant triggers. Application of therapeutic algorithm: attendees will master a stepwise treatment protocol, progressing from basic therapy (emollients / skin care, exposure reduction) to topical and systemic therapy, focusing on treatment choice through clinical cases and gaining confidence of using targeted therapies like delgocitinib."

JAK Inhibitors for Alopecia Areata: Approved Therapies, Efficacy, and Unanswered Questions.
(PubMed, Drug Des Devel Ther)
- "Baricitinib, a selective JAK1/JAK2 inhibitor, became the first FDA-approved systemic treatment for severe AA, followed by ritlecitinib, which targets JAK3 and TEC family kinases and deuruxolitinib targeting the JAK1/2. Other agents, including ivarmacitinib, and the topical agent delgocitinib, are under active investigation, offering varying selectivity profiles and routes of administration...Despite these remarkable advances, the field currently faces several significant challenges, including variable treatment responses, incomplete hair regrowth in patients with severe AA, potential long-term safety risks, and the absence of validated biomarkers to guide patient selection or predict therapeutic outcomes. Comparative efficacy, treatment challenges, and future directions including combination strategies and biomarker-guided therapy are also discussed to support informed clinical decision-making and highlight unmet needs in AA treatment."

Long-Term Safety of JAK and TYK inhibitors in the treatment of Psoriasis, Psoriatic Arthritis, and Atopic Dermatitis: A Systematic Review
(EADV 2026)
- "The search strategy combined terms related to Janus kinase inhibitors, including upadacitinib, abrocitinib, baricitinib, ruxolitinib, delgocitinib, tofacitinib, and "Janus kinase inhibitor"; TYK inhibitors, including deucravacitinib; adverse event terminology (e.g., adverse events, adverse effects, side effects, and safety); and disease indications (PsO, PsA, and AD). Conclusions JAK and TYK inhibitors demonstrate favorable efficacy with substantial rates of clinical response across PsO, PsA and AD, accompanied by a low incidence of serious adverse events. These findings support the overall benefit-risk profile of JAK and TYK inhibitors in immune-mediated inflammatory diseases while underscoring the importance of continued long-term safety monitoring in real-world populations."

Evaluation of delgocitinib cream in chronic hand eczema: Early Real-World Experience
(EADV 2026)
- "One experienced recurrent irritant-type flares requiring oral prednisolone, and another discontinued treatment after two tubes due to perceived lack of efficacy. The absence of COVID‑19 or nasopharyngitis likely reflects sample size and differing background infection rates (DELTA studies conducted during the COVID-19 pandemic). The observed plateau in HECSI improvement between weeks 4 and 16 may help clinicians counsel patients on expected treatment timelines and set realistic goals."

Topical pan-JAK inhibition with delgocitinib 20mg/g cream in inflammatory diseases of the nails: a real-life case series
(EADV 2026)
- "While topical data exist for molecules such as tofacitinib and ruxolitinib, evidence on delgocitinib, the newest topical pan-JAK inhibitor, remains limited, particularly regarding its off-label use in nail disorders...Its favorable cream vehicle formulation — non-greasy and rapidly absorbed — likely contributes to good patient adherence and tolerability, particularly in such a challenging anatomical site. In line with the shared pathogenic role of JAK-STAT signaling and existing evidence with other pan-JAK inhibitors such as tofacitinib, these findings further support the potential of this class of agents in improving inflammatory keratotic nail disorders and warrant confirmation in controlled clinical studies."

Molecular Transcriptomics-Guided Treatment Selection in a Case of Palmar Psoriasis
(EADV 2026)
- "Oral alitretinoin did not lead to relevant improvement. Oral ciclosporin resulted in a partial regression of the skin lesions, but no sustained complete remission. Additionally, topical delgocitinib cream as an add-on therapy also failed to achieve full symptom control...Based on this molecular profile, treatment with the IL-17A/F inhibitor bimekizumab was initiated subcutaneously...In this case, NanoString-based molecular profiling identified a Th17-dominant signature in a patient with diagnostically challenging chronic palmar inflammatory dermatosis and successfully guided treatment selection with IL-17A/F inhibition. This case supports the potential value of transcriptomic profiling as a practical decision-support tool in difficult to treat palmoplantar inflammatory dermatoses cases."

Treating successfully and keeping long-term remission in a patient with chronic hand eczema using delgocitinib
(EADV 2026)
- "Case Presentation A 47- year- old Caucasian female patient presented with redness, scaling, fissures, intense itching, and pain on both palms for the last 5 years, resistant to topical corticosteroids, calcineurin inhibitors, cyclosporine and alitretinoin. Our patient maintained the remission of disease for over a year by using the cream as a maintenance dose three times a week. Case reports like these suggest that studies with larger number of patients could in the future, help confirm the above benefits in daily clinical practice."
