Despite a significant overall decline in SLE-related mortality (AAPC −1.90%), progress stalled after 2009 and reversed during the COVID-19 pandemic in 2020. Critically, the 3.7-fold Black/White mortality disparity has not narrowed over 22 years of observation, and Southern states continue to record rates 2.5-fold higher than New England.
