Oral ciclosporin resulted in a partial regression of the skin lesions, but no sustained complete remission. Additionally, topical delgocitinib cream as an add-on therapy also failed to achieve full symptom control...Based on this molecular profile, treatment with the IL-17A/F inhibitor bimekizumab was initiated subcutaneously...In this case, NanoString-based molecular profiling identified a Th17-dominant signature in a patient with diagnostically challenging chronic palmar inflammatory dermatosis and successfully guided treatment selection with IL-17A/F inhibition. This case supports the potential value of transcriptomic profiling as a practical decision-support tool in difficult to treat palmoplantar inflammatory dermatoses cases."
