A Retrospective Comparison of Continuous Infusion and Intermittent IV Tacrolimus in Pediatric HCT (ASPHO 2026) - May 2, 2026 - "Baseline demographics were largely similar, though IIV patients were more frequently male (70.4% vs 50%), received letermovir (66.7% vs 22.2%), and received strong CYP3A4 inhibitors (78% vs 44%), reflecting evolving practice patterns...One PRES episode occurred in an IIV patient with hemoglobinopathy, recurring after conversion to sirolimus... In this retrospective analysis, CI and IIV tacrolimus yielded comparable therapeutic levels and safety outcomes in a pediatric HCT population. With no apparent differences in median levels or clinical outcomes, these findings support IIV as a feasible alternative to CI in this population. Intermittent IV tacrolimus administration may offer a practical option for centers seeking to reduce the operational burden of continuous infusion tacrolimus."
