Since most patients remained subtherapeutic after the initial adjustment, a higher empiric dose increase may be considered although should be individualized for patients with non-therapeutic levels at the time of VOR-D. As levels approach the therapeutic range, lab draw frequency may be gradually reduced to minimize patient burden."
