These findings highlight that subclinical cardiac involvement in APS may be more prevalent than clinically apparent, even in asymptomatic patients with no prior history of myocardial infarction. The slight elevation in native T1 in the APS group may reflect underlying fibrosis associated with the observed LGE abnormalities. Both LGE and perfusion findings are associated with higher risk of future cardiovascular events, and CMR offers a comprehensive tool to characterize this subclinical pathology."
