177 Lu-DOTATATE Therapy in Metastatic Adrenocortical Carcinoma: Lesion Dosimetry and Clinical Outcome (EANM 2026) - Sep 29, 2026 - Abstract #EP-1036; "Adjuvant chemotherapy with cisplatin and etoposide was administered; however, disease progression was observed and mitotane was subsequently initiated. This case suggests that 68Ga-DOTATATE PET/ CT may reveal a broader extent of disease than 18F-FDG PET/ CT in metastatic ACC; however, receptor expression alone may be insufficient to predict durable benefit from PRRT. Published data on SSTR-targeted radionuclide therapy in metastatic ACC remain scarce, particularly with respect to lesion-based dosimetry (1,2). In our patient, the discordant clinical course of pulmonary and abdominal metastases raises the possibility that pre-therapy dosimetric assessment may add meaningful information beyond receptor positivity alone, helping to refine lesion selection, treatment planning and therapeutic expectations."
