VEDOLIZUMAB OR USTEKINUMAB AND BILIARY DISEASE OUTCOMES IN PSC-IBD OVERLAP: A PROPENSITY SCORE– MATCHED COHORT ANALYSIS (AASLD 2026) - Oct 6, 2026 - Abstract #1181; Pres time: Nov 5, 2026; 12:00 PM - 01:00 PM; Location: Convention Center: Poster Hall, Hall DE; "Cohort 2 comprised PSC-IBD patients receiving conventional IBD therapy (infliximab, mesalamine, or azathioprine) without vedolizumab or ustekinumab. In this propensity-matched real-world cohort of PSC-IBD patients, vedolizumab/ustekinumab were associated with significantly lower need for ERCP and markedly lower all-cause mortality compared to conventional IBD therapy, without significant differences in ALP normalization, cholangitis, cirrhosis, or cholangiocarcinoma rates. Findings suggested that gut-selective immunomodulation may reduce biliary interventions and improve survival in PSCIBD, though the small cohort size limits causal interpretation. Prospective studies with dedicated biliary endpoints and ALP trajectory monitoring are urgently needed in this population."
