Talaromyces marneffei Infection Mimicking Refractory Pyoderma Gangrenosum in Ulcerative Colitis on Combination Immunosuppression (ACG 2026) - Aug 29, 2026 - Abstract #P5822; Pres time: Oct 13, 2026; 10:30 AM - 04:00 PM; Location: Exhibit Halls A - D (Level 3); "Case Description/ A 54-year-old man with severe ulcerative pancolitis on infliximab (10 mg/kg every 8 weeks), 6-mercaptopurine, and mesalamine presented with a progressively enlarging ulcerative lesion on the dorsal left hand one month after travel to Thailand...Initial treatment with topical corticosteroids failed, and immunosuppression was escalated with oral prednisone, high-potency topical clobetasol, topical ruxolitinib, and increased infliximab dosing to every 6 weeks without improvement...The patient was started on oral itraconazole 200 mg twice daily, resulting in rapid and marked clinical improvement...Lack of response to standard PG therapy should prompt early biopsy with microbial cultures, particularly in immunosuppressed patients with relevant travel exposure. To our knowledge, this represents the first reported case of a rare presentation of cutaneous talaromycosis mimicking PG in an IBD patient on biologic therapy."
